Dear Family Members,

The Board of Directors of Prospect Hill Cemetery offers our deepest sympathy and prayers for your grief.
At Prospect Hill Cemetery we will provide a burial and committal service with dignity.

The fees for burial and inurnment at the cemetery are identified on the accompanying list of service fees.
Once you have decided upon the type of service desired, you should ask the funeral director to remit
the completed burial authorization form with the appropriate fee to the cemetery.

Payment should be made to Prospect Hill Cemetery Association.

Once the cemetery has confirmed ownership, grave selection and the payment has been received,
the cemetery will work with the funeral director to schedule the committal service at the cemetery.
